     1. Lost or Stolen Debit / ATM Card
      To report a lost Debit / ATM card, please 1-800-528-2273, 24-hours a day, 7-days a week.
 
     2. Lost Password for Freedom eBanking
      If you have forgotten your Personal Online Banking password, or are experiencing complications
      logging in, please call one of our personal bankers during the hours of 8:30 am - 4:00 pm
     Monday - Thursday, 8:30 am - 7:00 pm Friday and 8:30 am - 3:00 pm Saturday.
 
     3. Lost Password for Business eBanking
     If you have forgotten your password for your individual ID for Business eBanking, please contact
     the Administrator at your company. If you have forgotten your password for the Administrator ID,
     or are experiencing complications logging in, please call your Internet banking specialist at your local branch,
     during normal banking hours.
     4. For 24-hour Telephone Banking
     If you have forgotten your password to access our 24-hour Telephone Banking, please call one of
     our personal bankers during the hours of 8:30 am - 4:00 pm Monday - Thursday, 8:30 am - 7:00 pm
     Friday and 8:30 am - 3:00 pm Saturday.
 
     5. Address Changes
     All address change requests must be made in writing. Please send your request to us by
     mail to: Foster Bank, Attention: Mail Room, 4001 W. Devon Ave., Chicago, IL. 60646 or
     by fax to 1.773.736.2081 (Attention: Mail Room).

     Please include the following information in your letter:
     1. Former address
     2. New address
     3. All of the account numbers affected by the address change.
     4. Signature form at least one authorized signer.
